Opportunities are provided for children and adults with developmental disabilities to lead healthy and fulfilling lives within the communities of Archuleta, Dolores, Montezuma, La Plata and San Juan counties. To begin the application process, contact a CCI office. Eligibility is determined by state regulations, which require using results of psychological testing which may indicate the presence of developmental disabilities including cognitive or other developmental delays. Once enrolled, the client is often eligible to pay by Medicated, Medicare or private insurance. All attempts will be made to assist the client in securing payment assistance. Guidance is provided in seeking eligibility for state payment.
Adult Programs: Daily living assistance, teaching independent living skills, assistance with job placement, job coaching, and other job-related activities
Family Support Services: Assistance and funding to families with children with developmental delays or disabilities with the goal of keeping the individual with disabilities living in the family home
Early Childhood Programs: Assessments for children under 3, assistance connecting families with treatment and therapy for early interventions

Recreational and team sports opportunities in Dolores and Montezuma counties provided for children and adults ages 7-70 with various disabilities – mental and physical – throughout the year. People wishing to work with the disabled are encouraged to participate as volunteers. There is no cost for the program except for a willingness to participate and take time to have fun with others.

“Cross-disabilities” means that an individual’s disabilities present no limitations or boundaries to the individual as a person and consumer. Programs are provided to person in the Four Corners states of Colorado, Arizona, Utah and New Mexico with a focus on rural areas. Support is provided for families and friends of all cultures and all forms of sexual preference. All information and referrals are held in strictest confidence.

Assistance is provided in the acquisition of eyewear and other vision assistance devices for all members of the community not otherwise able to afford them. Sight screening is provided for preschool children, and additional funds may be disbursed as available.
