Home health and hospice care are provided to patients and their families within a 70-mile radius of Cortez. Team concept to care is provided by nurses, certified nurse aides, medical director, therapists, social worker, volunteers, etc. Programs support physical, emotional, and spiritual needs of terminally ill patients and their families through interdisciplinary teams which include patient and family members. Patients are helped to maintain and enhance quality of life while remaining in the setting of their choice, pain-free, in control, and in touch with family and friends. Referrals can be made by anyone including doctors, hospital, family, or friends. Medicare, Medicaid, and private insurance are all accepted.

Housing assistance provided to low-income families, the disabled, and the elderly in Dolores, La Plata, and Montezuma counties. Eligibility is determined by total family income. Applications are initiated in person at agency office, and require information on income and family size. Contracts are normally written with one-year lease.

Promotion of a socially and economically balanced community is sought through provision of assistance and services to very-low to moderate income families, individuals, elderly, disabled, and special needs populations in areas including, but not limited to, housing and energy in Archuleta, Dolores, La Plata, Montezuma, and San Juan counties. Applications are available for all programs in the office or by mail.
Homeowner Rehabilitation Program: Housing rehabilitation for low to moderate income owner occupants in the five county region, funded through the Colorado Division of Housing, Community Development Block Grant.
Weatherization Program: Provides energy conservation measures for low income owners and renters in the five county region. There is no cost to recipients. The services provided may include insulation, furnace checks, appliance replacement, and energy audits.
Rental Assistance: Provides qualified families with the opportunity to live in safe, decent and sanitary rental housing in Archuleta, La Plata and San Juan counties through the Housing and Urban Development (HUD) Housing Choice Voucher program. The program is designed for families to pay 30% of their income towards rent and utilities.
Transitional Housing: Homeward Bound facility provides housing for homeless families with children
Scattered Site Self-Sufficiency: Provides subsidies for homeless families with children so the family can enter into a lease with a landlord
Emergency Homeless Prevention Program: Provides emergency housing funds for low-income families in the five county region facing homelessness due to a temporary interruption in income. These funds are available for rental, mortgage, and utility payments.

Parenting classes to increase positive parenting skills are offered regularly. They are 12-weeks long and include dinner and childcare. Children learn social and emotional skills in early childhood settings during Dinosaur School. Call for a class schedule.

Vocational training and education opportunities are provided to youth between ages 16-24 that they would not otherwise be able to afford. The focus of Job Corps is to increase each person’s employability. Services are available to residents of Archuleta, Dolores, La Plata, Montezum, San Juan and San Miguel counties. Eligibility is based on income and is determined on a case-by-case basis. The application process is by appointment and conducted in a personal interview setting. Free education and training to those who qualify.
