No Image AvailableThe center works with people with disabilities in Archuleta, Dolores, La Plata, Montezuma and San Juan counties to provide assistance choosing and directing long-term care supports, developing independent living skills, and accessing community resources and public benefits that fit with goals identified through person-centered planning.
Information and Referral: For benefits, services, and resources available to support independent living goals
Advocacy: On behalf of clients with agencies for access to services and resources that support independent living goal, physical access in public sector and vocational services; systemic advocacy to promote state and national programs, projects and benefits that create more opportunity for people with disabilities to choose and direct their own supports
Independent Living Skills Training: Cooking, self-advocacy, personal care, etc
Housing: Provision and management of HUD Section 8 housing voucher program; systemic advocacy to promote accessible, affordable, integrated housing and collaborative work with other housing agencies and organizations
Low Vision Assessments & Assistance: Promoting access to assistive technology devices and programs for people with low vision – particularly for seniors; facilitation and development of workshops, expositions, and demonstrations of assistive technology devices and equipment
Recreation: Bringing people with disabilities together to plan social outings and fun activities in the community, assistance with arranging logistics and facilitation of events
Employment: Ticket to Work vouchers accepted from clients who receive Social Security Administration benefits, assistance to beneficiaries in securing jobs and maintaining benefits
Community Transition Services: Assistance for residents of care facilities to identify goals for living independently with supports in the community, access to resources and assistance with choosing and directing supports to live safely and securely at home
Hours: 9 AM – 5 PM Monday through Friday
