No Image AvailableA variety of health care services, health education, sanitation, food service inspection, and environmental health services are provided to the residents of Montezuma County. Some services are also provided for Dove Creek. Many services are free.
CDEC: Child Development Evaluation Clinic
Communicable Disease Control Program: To prevent and monitor communicable diseases
Environmental Health: The sanitarian is responsible for monitoring environmental factors which affect the health of the community
Handicapped Children’s Program: Evaluation and treatment for children from birth to 21 years of age who have a physical disability which interferes with normal growth and development
Immunization Clinic: Routine immunizations provided free to low income families
Nurse-Family Partnerships: Support for first-time mothers
PATH (Partners in Action for Teen Health): A multi-disciplinary community based team coordinates resources and activities for adolescents in Montezuma County
Prenatal Program: Prenatal care is provided to non-Medicaid clients who meet financial guidelines. Care is provided in conjunction with area physicians.
Well Child Clinic: A clinic for children up to the age of five who meet financial guidelines
WIC (Women, Infants and Children): Provides supplemental foods and nutrition education at no cost to women, infants and children who meet financial and nutritional risk factor guidelines
Options for Long Term Care: A case management program for Medicaid Long Term Care clients also provides resources for private pay clients
Personal Care Provider Agency: Homemaking/personal care for financially/medically eligible
Healthy Kids-Nurse Family Partnership: Home visitation first time, low income eligible
