No Image AvailableOpportunities are provided for children and adults with developmental disabilities to lead healthy and fulfilling lives within the communities of Archuleta, Dolores, Montezuma, La Plata and San Juan counties. To begin the application process, contact a CCI office. Eligibility is determined by state regulations, which require using results of psychological testing which may indicate the presence of developmental disabilities including cognitive or other developmental delays. Once enrolled, the client is often eligible to pay by Medicated, Medicare or private insurance. All attempts will be made to assist the client in securing payment assistance. Guidance is provided in seeking eligibility for state payment.
Adult Programs: Daily living assistance, teaching independent living skills, assistance with job placement, job coaching, and other job-related activities
Family Support Services: Assistance and funding to families with children with developmental delays or disabilities with the goal of keeping the individual with disabilities living in the family home
Early Childhood Programs: Assessments for children under 3, assistance connecting families with treatment and therapy for early interventions
Hours: 8 AM – 5 PM Monday through Thursday
